Giant Left Ventricular Aneurysm Mimicking Myocardial Bridge

      Abstract

      An 81-year-old woman was admitted due to acute pulmonary edema. The electrocardiogram showed new Q-waves in anterior leads with ST-elevation in anterolateral leads. Coronary angiogram showed a 100% occluded second diagonal branch with an image of systolic and diastolic compression of the mid-to-distal portion of the left anterior descending coronary artery (LAD) causing a long “bridgelike” effect. Ventriculography revealed a giant aneurysm of the anterolateral wall. A 3-dimensional volume–rendered cardiac computed tomography scan suggested that the aneurysm produced extrinsic compression of the LAD.

      Résumé

      Une femme de 81 ans a été admise en raison d'un œdème pulmonaire aigu. L'électrocardiogramme a montré de nouvelles ondes Q dans les dérivations antérieures en plus d'un sus-décalage du segment ST dans les dérivations antérolatérales. La coronarographie a montré une occlusion de la deuxième branche diagonale avec l'image d'une compression systolique et diastolique de la portion moyenne à distale de l'artère coronaire descendante antérieure gauche (DAG) causant un long effet de « pont ». Une ventriculographie a révélé un anévrisme géant de la paroi antérolatérale. Une tomodensitométrie cardiaque a suggéré que l'anévrisme avait produit une compression extrinsèque de la DAG.
