Emergency Transcatheter Aortic Valve Implantation for Acute and Early Failure of Sutureless Perceval Aortic Valve

      Abstract

      We report the case of a 78-year-old woman admitted for cardiogenic shock related to acute and early failure (severe aortic regurgitation) of a Perceval sutureless aortic bioprosthesis (Sorin Group, Saluggia, Italy). Clinical stability was achieved using rescue transfemoral transcatheter aortic valve-in-valve implantation with an Edwards SAPIEN 3 prosthesis (Edwards Lifesciences, Irvine, CA). To our knowledge, we report herein the first case of successful valve-in-valve implantation using a SAPIEN 3 transcatheter heart valve in a sutureless bioprosthetic aortic valve with acute and early deterioration.

      Résumé

      Nous rapportons le cas d’une femme de 78 ans admise pour un choc cardiogénique lié à l’échec rapide et précoce (régurgitation aortique grave) d’une bioprothèse valvulaire aortique sans suture Perceval (Sorin Group, Saluggia, Italie). L’implantation d'une valve aortique de sauvetage dite valve-in-valve par cathéter par voie transfémorale au moyen de la prothèse Edwards SAPIEN 3 (Edwards Lifesciences, Irvine, CA) a permis d’atteindre une stabilité clinique. À notre connaissance, il s’agit ici de la première implantation dite valve-in-valve réussie d’une valve cardiaque percutanée ou par cathéter SAPIEN 3 dans une bioprothèse valvulaire aortique sans suture ayant subi une détérioration rapide et précoce.
