Abstract
The application of transcatheter aortic valve implantation (TAVI) in a wider and younger
population in the near future makes the need to successfully deal with challenging
procedural conditions an imperative. We report a case in which TAVI was successfully
conducted in a patient with aortic coarctation using a last-generation low-profile
bioprosthesis. Despite the fragile aortic wall and its possible abnormal function,
a low-profile (14F) CoreValve Evolute R device (Medtronic, Minneapolis, MN) was implanted
through the coarctation without complications. Last-generation, low-profile transcatheter
implanted aortic valves provide an additional advantage in patients with structural
impediments.
Résumé
L’implantation valvulaire aortique par cathéter (IVAC) chez un plus grand nombre de
patients et chez des patients plus jeunes se fera de plus en plus courante. C’est
pourquoi il est impératif d’arriver à pratiquer cette intervention même dans des conditions
difficiles. Nous présentons un cas où une IVAC a été réalisée avec succès chez un
patient qui présentait une coarctation de l’aorte. En dépit d’une paroi aortique fragile
et dont le fonctionnement était possiblement compromis, l’équipe a été en mesure d’implanter
une bioprothèse faiblement profilée (low-profile) de toute dernière génération, soit un dispositif CoreValve Evolute R (14F) (Medtronic,
Minneapolis, Minnesota) en passant par la coarctation sans que cela n’entraîne de
complications. L’implantation de ces nouvelles valves aortiques offre un avantage
additionnel chez les patients qui présentent des problématiques structurelles.
