Abstract
Transapical valve-in-valve mitral valve implantation (TA-MVI) has been described as
an alternative treatment option in patients with degenerated bioprostheses in the
mitral position. We report the first case of prosthesis dislocation and migration
into the left atrium after TA-MVI. A new prosthesis was implanted using the same approach.
The dislocated prosthesis was successfully removed through the left atrial appendage
through an extended anterolateral thoracotomy without the use of cardiopulmonary bypass.
Résumé
L’implantation valvulaire mitrale de type valve-in-valve par voie transapicale (IVM-TA) a été décrite comme une solution de rechange au traitement
des patients porteurs d’une bioprothèse dégénérée en position mitrale. Nous rapportons
le premier cas de déplacement et de migration de prothèse dans l’oreillette gauche
après l’IVM-TA. Une nouvelle prothèse a été implantée selon la même approche. La prothèse
mal positionnée a été retirée avec succès par l’appendice auriculaire gauche par thoracotomie
antérolatérale étendue sans l’utilisation de pontage cardiopulmonaire.
