Aortitis With Associated Left Main Coronary Artery Stenosis: Role of Cardiac Computed Tomography

      Abstract

      We report on a 49-year-old man who presented to the emergency department with progressive angina. Echocardiography displayed severe aortic regurgitation and aortic valve thickening. The suspected diagnosis was acute aortic syndrome. Cardiac computed tomography showed circumferential thickening of the aortic wall and left main coronary artery ostial stenosis. Histologic examination showed diffuse aortic inflammation. No damage of any other organ or vascular structure was reported, and the final diagnosis was nonspecific aortitis. Differential diagnosis, prognosis, and therapeutic strategies are discussed.

      Le rapport fait état d’un homme de 49 ans atteint d’angine évolutive qui s’est présenté au service des urgences. L’échocardiographie a révélé une régurgitation aortique sévère et un épaississement de la paroi de l’aorte. Un diagnostic de syndrome aortique aigu a été évoqué. La tomodensitométrie cardiaque a mis en évidence un épaississement circonférentiel de la paroi de l’aorte ainsi qu’une sténose ostiale de l’artère coronaire gauche principale. L’examen histologique témoignait d’une inflammation diffuse de l’aorte. Selon ce qui a été rapporté, aucune autre structure organique ou vasculaire n’était touchée. Un diagnostic définitif d’aortite non spécifique a été posé. Le diagnostic différentiel, le pronostic et les stratégies thérapeutiques sont abordés.
