Catheter-Based Palliation in an Infant With Obstructed Cor Triatriatum

      Abstract

      A 33-day-old infant with obstructed cor triatriatum sinister and partial anomalous pulmonary venous drainage presented with respiratory distress and fever. Her suprasystemic pulmonary hypertension was relieved by opening the connection to the right atrium using balloon atrial septoplasty and septostomy, and to the inferior chamber using balloon dilation of a fenestration in the dividing membrane. This enabled extubation and discharge, with elective surgical repair at 2 months. To our knowledge, this is the youngest patient to receive a catheter intervention for obstructed cor triatriatum sinister, providing relief of pulmonary hypertension and postponement of surgical repair.

      Résumé

      Un nourrisson de 33 jours ayant un cœur triatrial gauche obstructif et un retour veineux pulmonaire anormal partiel a présenté une détresse respiratoire et de la fièvre. L’hypertension pulmonaire suprasystémique qu’elle manifestait a été soulagée par l’ouverture de la connexion à l’oreillette droite à l’aide d’une atrioseptoplastie et d’une atrioseptostomie par ballonnet, et à la chambre inférieure à l’aide de la dilatation par ballonnet d’une fenestration dans la membrane de séparation. Cela a permis l’extubation et la sortie d’hôpital, ainsi que la correction chirurgicale non urgente à 2 mois. À notre connaissance, il s’agit de la plus jeune patiente à recevoir une intervention par cathéter pour un cœur triatrial gauche obstructif, qui a procuré le soulagement de l’hypertension pulmonaire et a permis le report de la correction chirurgicale.
