Abstract
Spontaneous coronary artery dissection is an uncommon nonatherosclerotic cause of
acute coronary syndrome. It usually occurs in young women and is often associated
with fibromuscular dysplasia, connective tissue diseases, and pregnancy or postpartum
states. We present a case of a Syrian woman with a history of grand multiparity and
recent miscarriage who presented with non–ST-elevation myocardial infarction and was
found to have multivessel spontaneous coronary artery dissection and severe left ventricular
dysfunction.
Résumé
La dissection spontanée de l’artère coronaire est une cause rare d’origine non athéroscléreuse
du syndrome coronarien aigu. Elle apparaît généralement chez les jeunes femmes et
est souvent associée à la dysplasie fibromusculaire, aux connectivites et à la grossesse
ou au post-partum. Nous présentons le cas d’une femme syrienne ayant des antécédents
de grande multiparité et de fausse couche récente qui a présenté un infarctus du myocarde
sans sus-décalage du segment ST et chez qui nous avons observé une dissection spontanée
de l’artère coronaire de type multitronculaire et une dysfonction grave du ventricule
gauche.
