Successful Treatment of Type 1 Cryoglobulinemic Vasculitis With Cardiac Involvement

      Abstract

      Cryoglobulinemic vasculitis is a rare and frequently fatal type of myocarditis. Cardiac manifestations in type 1 cryoglobulinemic vasculitis have never been reported to our knowledge. We report a rare case of type 1 cryoglobulinemic vasculitis with cardiac involvement in a patient who experienced progressive heart failure during the diagnosis. The diagnosis was made by the presence of cryoglobulins and endomyocardial biopsy results. After bortezomib-containing treatments, plasma cryoglobulin levels returned to normal, and the patient's clinical condition gradually improved.

      La vascularite cryoglobulinémique est une forme de myocardite rare, mais souvent fatale. À notre connaissance, les manifestations cardiaques de la vascularite cryoglobulinémique de type 1 n’ont jamais été rapportées. Nous faisons état d’un cas rare de vascularite cryoglobulinémique de type 1 associant une atteinte cardiaque chez un patient qui souffrait d’insuffisance cardiaque progressive lors du diagnostic. Le diagnostic a été posé en raison de la présence de cryoglobulines et des résultats de la biopsie endomyocardique. Après les traitements à base de bortézomib, les concentrations plasmatiques en cryoglobuline revenaient à la normale, et l’état clinique du patient s’est graduellement amélioré.
