Abstract
Most allergic reactions to radiocontrast media can be managed with a pretreatment
protocol of steroids and antihistamines. However, in rare circumstances, patients
can have severe allergic symptoms despite pretreatment. In this case, we highlight
the use of a novel desensitization protocol in a patient with a severe breakthrough
contrast allergy and unstable angina that allowed for successful percutaneous coronary
intervention. The protocol was well tolerated, feasible, and might allow other patients
with similar allergies to undergo angiography and imaging studies successfully that
require the use of radiocontrast media.
Résumé
Dans la plupart des cas, les réactions allergiques aux produits de contraste radiologique
peuvent être prises en charge au moyen d’un protocole de prétraitement comprenant
des corticostéroïdes et des antihistaminiques. Il arrive toutefois dans de rares cas
que le patient présente malgré tout des symptômes sévères d’allergie. Ici, les auteurs
décrivent un nouveau protocole de désensibilisation grâce auquel une intervention
coronarienne percutanée a pu être pratiquée avec succès chez un patient présentant
une angine instable et ayant eu une forte réaction allergique au produit de contraste
malgré le prétraitement. Le protocole était réalisable et a été bien toléré, et il
pourrait permettre à d’autres patients souffrant d’allergies similaires de subir avec
succès une angiographie et des examens d’imagerie nécessitant l’emploi d’un produit
de contraste radiologique.
