Rubidium-82 Positron Emission Tomography With Intravenous Ergonovine to Diagnose Vasospastic Angina

      Abstract

      The gold standard to diagnose vasospastic angina is intracoronary reactivity testing, which is performed selectively at dedicated centres. Noninvasive imaging with single-photon emission computerized tomography (SPECT) or echocardiography does not enable accurate localization of spasm or quantification of change in myocardial perfusion in response to an abnormal vasoreactivity. Rubidium-82 positron emission tomography myocardial perfusion imaging (82Rb PET-MPI) with intravenous ergonovine was used to diagnose refractory vasospastic angina in a patient with a complex ischemic syndrome, recent coronary stenting, and persistent atypical angina despite maximal tolerable doses of guideline-directed medical therapy.

      Le test de référence pour diagnostiquer l'angine vasospastique est le test de réactivité intracoronaire, qui est réalisé de manière sélective dans des centres spécialisés. L'imagerie non invasive telle que la tomographie par émission monophotonique assistée par ordinateur (SPECT) ou l'échocardiographie ne permet pas de localiser précisément le spasme ni de quantifier la modification de la perfusion myocardique en réponse à une vasoréactivité anormale. L'imagerie de perfusion myocardique par tomographie par émission de positons avec le rubidium-82 (TEP-Rb) avec ergonovine en intraveineuse a été utilisée pour diagnostiquer une angine vasospastique réfractaire chez un patient présentant un syndrome ischémique complexe, une pose récente d'une endoprothèse coronaire et une angine atypique persistante malgré les doses maximales tolérables du traitement médical recommandé.
