      My next patient is wearing an orange jumpsuit. His hands are cuffed to a chain around his waist and the chain between his ankle cuffs is short, so that if he tried to run away he would not get very far. His shoulders are slumped and his eyes downcast. He is accompanied by a large deputy sheriff with his surname embossed on his spotless uniform, packing a large spotless handgun in a holster at his large waist.
